Frank Pierce (Nicolas Cage) je na dně. Po pěti letech vyčerpávajících nočních služeb v sanitce záchranky se z něho stal alkoholik trpící těžkou depresí a halucinacemi - duchové desítek pacientů, které se mu nepodařilo zachránit, bloumají ulicemi Manhattanu s němou výčitkou nevyslovitelného utrpení. Záblesk naděje vnese do Frankova vyprahlého světa tajemná dívka Mary Burkeová (Patricia Arquette), jejíhož otce Frank vyrval z náručí smrti.
